Self Referral Services
If you come to the practice to discuss an issue you have, some patients will be referred to another service. You can now self-refer to a lot of services in the area, removing the need for a GP appointment and cutting down the waiting time for you.
These services include,
- New Pregnancy
- Anxiety, stress, and Mental Health
- Contraception and Sexual Health
- Bones, Joints, and Muscular pain (MSK)
- Audiology
- Podiatry
- Wheelchair Services
- Community Equipment Services

To request an urgent, same day appointment (Monday to Friday)

When you get in touch, we’ll ask what you need help with. We will use the information you give us to choose the most suitable health professional to help you and decide if you require a face to face or telephone appointment.
To request a routine appointment for a GP in the next 14 days

If you have a non-urgent problem or you don’t need to be seen face to face you can submit a request for an appointment online a member of the team will review the request within 48 hours and direct the problem appropriately. Alternatively, you can call the Practice during our opening times on 01429 278179.

We may then offer you a consultation:

- Face to face at the surgery
Patients can decide if they want a face to face or telephone appointment. You will be informed by the practice if your request is unsuitable for a telephone consultation.
To request a Nurse or Health Care Assistant (HCA) appointments
Nurse and Health Care Assistant (HCA) appointments must be booked through a member of our team by calling the Practice on 1429 278179 this ensures you are booked with the correct Nurse and for the correct amount of time.
Cancelling or changing an appointment
If you cannot make your appointment let us know as soon as you can so another patient can book the appointment time. You can cancel or change your appointment:

If you need help when we are closed
- Use NHS 111 online (111.nhs.uk) or call 111. 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
- If it’s a life-threatening emergency call 999.
- For urgent medical advice from 8.00am to 8.30am please press 9
- If you are calling between 6.00pm to 6.30pm Monday to Friday and require advice, please call 07392 866847. Please note, you will not be able to book appointments on this number.
- Evening and weekend appointments are available from 6.30pm to 8.00pm Monday to Friday, 10.00am to 5.00pm Saturdays and Sundays 11.00am to 4.00pm. For further information see below.
The evening and weekend appointments are available to everyone registered with a GP in Hartlepool and Stockton-on-Tees and patients can book an appointment by calling their GP Practice or NHS 111.

- Tennant Street Medical Practice, Stockton: 10.00am to 1.00pm

Sunday
- Chadwick Practice, One Life Centre, Hartlepool: 11.00am to 1.00pm
- Tennant Street Medical Practice, Stockton: 11.00am to 1.00pm
- Woodbridge Practice, Ingleby Barwick: 2.00pm to 4.00pm
Home Visits
To request a home visit, contact the surgery on 01429 278179 before 10.30am.
Remember that we can see several patients at the practice in the time it takes for one home visit. Patients should have their appointment at the surgery if they can.
You may only request a home visit if you are housebound or are too ill to visit the Practice, visits requested will be passed to the duty GP and should be of an urgent nature only.
You can also be visited at home by a Community Nurse if you are referred by your GP. You should also be visited at home by a Health Visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Lack of transport is not a reason to request a home visit.
